Cardiometabolic Medicine

Hypercholesterolaemia, triglycerides, diabetes and hypertension are significant factors in cardiovascular disease. This includes heart attacks, strokes and peripheral vascular disease. A holistic approach in management of these conditions is key to preventing events. Dr Mbagaya’s experience allows him to practise as one of the few cardiometabolic specialists.

Dr Mbagaya undertakes the Investigation and treatment of patients with high cholesterol to identify causes and create a personalised care plan for the patient. His personalised approach allows him to prepare individualised targets for his patients. He also reviews patients with side effects of statins and patients unable to achieve targets. He works collaboratively with patients to identify barriers to achieving goals.

Dr Mbagaya offers comprehensive markers of cardiovascular risk not commonly offered such as Lp(a), ApoB, CRP and others on personal request. He identifies individual patients’ suitability to use newer medications such as injectable medicine for cholesterol while consulting closely with a dietitian who can address lifestyle changes. He works closely with cardiologists to identify and manage any long term effects of hypercholesterolaemia.

He also sees patients with cholesterol disorders related to medications, eating disorders or other medical conditions which have been difficult to diagnose. His clinical acumen allows him to identify otherwise missed causes of high cholesterol.

Dr Mbagaya has extensive experience in the management and follows up of patients with raised triglycerides and mixed dyslipidaemia. He follows up patient for prevention of heart attacks, strokes and peripheral vascular disease. This involves the understanding of an individual patient’s metabolic profile. He has specialist interest in cardiometabolic risk in women, menopause, autoimmune disease, previous chemotherapy and fatty liver disease.

Prediabetes and Metabolic Syndrome

Metabolic Syndrome and Prediabetes are major causes of cardiovascular disease and future diabetes. Patients with metabolic syndrome may present with raised triglycerides, high blood pressure, abnormal cholesterol and triglycerides, high blood sugar and fat around the waist. This risk increases with age, presence of obstructive sleep apnoea, non alcoholic fatty liver disease, polycystic ovarian syndrome. Some patients with metabolic syndrome may first present with conditions such as kidney stones.

Dr Mbagaya works closely with individuals to help them avoid cardiovascular disease and the complications that come with metabolic syndrome and prediabetes. He has a keen interest in men’s health which may present with sleep difficulties, kidney stones, erectile dysfunction or low energy. A thorough medical history and examination may reveal underlying medical issues.

Blood Tests

While blood tests are frequently requested by clinicians, they must be interpreted in the proper context. Dr Mbagaya provides guidance and clarity to patients on the interpretation of blood tests, the selection of blood tests, and the assessment of blood tests.

He also provides vitamin deficiency tests, thyroid disease screening, targeted health screening and fatigue investigation while recommending specific diagnostic panels to his patients. He guides patients on the investigation and monitoring of heavy metal poisoning.

The proliferation of easy access blood tests means that patients can access many blood tests without the necessary clinical support. These include genetic panels, metabolic panels and other screening tests. This leads to increased anxiety, uncertainty and unwarranted further testing. A metabolic nutritional deficiency occurs when your body does not get the nutrients it needs to function properly. Many individuals with vitamin and mineral deficiencies wait too long to be diagnosed and treated. This can have an adverse impact on their lives. Vitamin and mineral deficiencies may occur after periods of hospitalisation, extensive surgery, chemotherapy treatment or poor appetite. Patients on newer immunosuppressants and immunotherapy are especially at risk of this. Such patients are recommended to undergo specific testing and personalised supervision under Dr Mbagaya’s expertise.

Patients planning substantial life changes and health changing medical treatments such as pregnancy, IVF treatment, or surgery may require metabolic optimisation. This ensures that they are in optimal health for such changes.  Similar factors are faced by patients following major surgery, chemotherapy or prolonged hospitalisation.
